For your convenience and to better your overall patient experience, we have made a number of items available for you online. To help reduce your wait time, please bring all necessary paperwork to your appointment as this will help to expedite your check-in process. Please download, print & complete any forms that are required for your appointment.
Preparing For Your Appointment
Please bring the items listed below on the day of your appointment and be prepared to pay any co-pay or deductible applicable to your insurance plan. Our staff is happy to assist you before your appointment in determining if we area participating provider with your insurance plan.
- Your Government Issued Identification
- Insurance information
- The completed forms (see below)
- List of all medications you are currently taking
- Recent copies of x-rays and lab results
Upon arrival, a front office staff will provide you with any forms needed to get you checked in quickly. Once checked in our staff will call for you as soon as possible and will also inform you if any wait time exceeds 15 minutes.

Arrival Time
New patients are asked to arrive 15 minutes early to complete paperwork, and allow time so that staff can verify and complete your medical record. We encourage you to download, print, and complete the forms prior to your appointment. Established patients should check-in 10 minutes prior to their appointment as we make every effort to stay on schedule, although emergencies do arise.
Cancellation Policy
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule your appointment. Not only will you avoid a late cancellation charge, this will allow us to accommodate other patients requesting an appointment. If you are running 15 minutes late of your original appointment time, we may need to have you rescheduled to ensure that other patients will be seen for their appointment.
